  1. Agreement upon Reopening Post Covid-19: Working Out Together
  2. LEVEL Fitness has put preventative measures in place to reduce the spread of Covid-19 including monitoring and reducing club capacity, fewer entry touch points, 6ft spacing between equipment and regular, frequent, disinfecting and sanitizing. We would like each member to acknowledge the contagious nature of Covid-19 and agree to work together to protect yourself, our staff and other members.Members must adhere to CDC/State/Local Government Guidelines as well as LEVEL Fitness policies below. Every member will need to complete the Member Liability form in order to resume working out at LEVEL Fitness. Parents of Minor Members will need to complete a Minor Member Liability Form for their underage child.
  4. You acknowledge member responsibilities include:
    1. Come Healthy- self screen before entering the club for any signs of illness, including fever for the past 14 days
    2. The club capacity is LIMITED and my admittance is not guaranteed or I may be asked to wait to gain access to workout
    3. Check in at the Front Desk so we can track capacity
    4. Maintain Social Distancing- keeping 6 feet apart
    5. Disinfect all equipment you touch before and after use
    6. Wash hands frequently with soap for 20 seconds
    7. Wear masks according to CDC and NYS Guidelines
    8. Compliance with the new directional signage in the club 
  5. MEMBER CONSENT*By signing this liability waiver, I fully acknowledge the infectious nature of COVID-19 and voluntarily accept the risk of potential exposure to or infection by COVID-19. I understand that the risk of becoming exposed to and/or infected by COVID-19 at LEVEL Fitness may result from active or passive negligence of myself or others, including but not limited to, LEVEL Fitness employees, its successors or assigns, as well as, its officers, agents, other members and/or other guest. I agree to assume all risk and accept sole responsibility for any injury to myself and/or my child, illness, damage, loss, claim, liability, or expense, of any kind. I further agree to indemnify and hold harmless LEVEL Fitness, its employees and representatives from any and all damages resulting from me and/or my guest entering LEVEL Fitness and whether a COVID-19 infection occurs before, during, and after visiting LEVEL Fitness.
